If you did not choose all five score recipients on test day, or if you wish to send your GMAT® scores to more than five schools, you may use the Additional Score Report Request Form to submit your request.
You may submit the Additional Score Report Request form by the following methods—phone, fax, or mail. If submitting the form by phone or fax, you must pay by credit card. Visa, MasterCard, JCB® and American Express are accepted. If you are mailing the form, please include a check, or a money order, or credit card information with your form. Requests received without payment will be returned.
You may submit the form to GMAT® Customer Service in your region.

What You'll Need
Additional Score Report (ASR) Request Form is an
Acrobat PDF (Portable Document Format) file. PDF files are viewed with free
Acrobat Reader software from Adobe.
If you do not already have Acrobat Reader installed on your computer, you'll
need to download and install it. You can do this by clicking on the "Get
Acrobat Reader" button below. A new window will open with instructions for
downloading and installing Acrobat Reader.
Acrobat Reader is available for Windows, Macintosh, and a variety of other
computer platforms.
