Will I receive confirmation that my request form and supporting documentation have been received?

Yes. However, you should keep copies of all communications sent to GMAT® Disability Services for your records. You should put your name and date of birth on each page of documentation that you submit for review (on the front and back if page is two-sided). This includes all fax and email submissions.
