What must I submit in order for my request for accommodations to be reviewed?

Requests for accommodations will not be reviewed until you have submitted the following documents:

• A completed GMAT Test Accommodation Request Form
• The test fee as specified on mba.com
• Disability documentation appropriate to your disability, as described in this Supplement
